Dear Future Preschool Parent, 

Applications are now being accepted for the upcoming 2011- 2012 Fall 3K & 4K programs. 

Please take a few minutes to look over the packet of information, including our 3K/4K handbook, which helps explain our preschool program and answers many of the questions you might have. 

If you would like to register your child for either our 3K or 4K programs, please fill out the registration form, health forms, emergency information and physical form. The physical form will need to be signed by your child’s pediatrician, so please keep this in mind.
 
One other thing, we ask that you bring along a certified copy of your child’s birth certificate to verify their age.  Once we see this document, a simple form will be filled out by our office and kept in your child’s file. The birth certificate will be returned to you once the form is completed. 

Please return all the completed 3K/4K registration forms to our center with the non-refundable $25.00 registration fee.   Once our center receives your forms and payment, then your child’s spot will be secured in either our 3K or 4K program for the days and times you have chosen for the upcoming school year. 

We ask that you return your forms early, since our programs fill up quickly. 

If you are also applying for the school CHOICE program, please complete, date and sign the forms and return them to our center with the proper documentation.  The open enrollment period runs through the 20th of the month, so please return your CHOICE application form as soon as possible. There are a limited number of CHOICE seats available. 

We also ask that all the enrolled students and their parents plan on attending St. Lucas Elementary School Registration Day in early August.   Additional information will be given to you about our preschool program, as well as offering you a time to sign-up for the FACTS payment plan (4K) and make any payments for tuition, hot lunch and/or milk (3K & 4K). Tours of the classrooms will also be available that day.

Goal
The overall goal of our 3K & 4K preschool programs at St. Lucas Preschool, is to provide a Christ centered learning environment for your child, which offers daily experiences that nurture the spiritual, intellectual, physical, social and emotional development of your child. 
The St. Lucas Lutheran preschool provides Christian education on a non-profit basis for the members of our congregation, for members of other WELS/ELS congregations, and prospective members in our community.  All children are welcome without reference to race, national or ethnic origins, gender, or ability.  Children with special needs are accepted if, in the judgment of the staff, the program is in the best interest of the child and no extra staff is required. 

Program
We believe that through daily Bible Stories, fingerplays and songs your child will learn to know the love of their Savior, Jesus Christ.  As God’s word reminds us, “Where two or three are gathered together in my name, there am I in the midst of them.”  Jesus is the center of our classroom environment and we guide the children throughout the day, to make God pleasing decisions in their play and interaction with other children in our classroom.

Our program also encourages a “creative curriculum” with age-appropriate hands-on learning in math, science and writing to help develop your child’s fine and large motor skills. Your child will use many types of learning materials throughout the year such as flashcards, learning games, manipulative, learning pages and literature to develop your child’s pre-reading and writing skills. These activities will also help to increase your child’s  math concepts and number recognition.

Social interaction is a very important part of our curriculum and is encouraged through center play, skits, puppet shows and art & craft projects to help develop your child to his or her fullest learning potential, thereby becoming ready for the Kindergarten experience.

When is the 3K & 4K Preschool Program Closed?

The 3K & 4K program is closed when St. Lucas Elementary grade school is closed due to holidays, vacation periods, teacher conventions or teacher in-service days. (Our “No School Day” Care program is open for your convenience during these closed days.)

The Entire Center will be closed on the following Holidays: (This includes the childcare also) New Year’s Eve, New Year’s Day, Good Friday, Memorial Day (observed), Independence Day, Labor Day, Thanksgiving and the day after, Christmas Eve, and Christmas Day.  Other possible center closings due to weather or other reasons will be announced throughout the year.

The “NO SCHOOL DAY” Program

Throughout the school year, the elementary school is closed for various reasons.  During these ”No School Days” the 3K & 4K program is also closed.  Your child (ren) then may choose to attend the “No school Day“ program that is offered during school breaks for children ages 3 through age 12 , which is held in our before and after school classroom.
If your child needs such care, you must register your child in advance, using a “ No School Day” form, which will be given to the parents throughout the school year. 
If you do decide to use this program, there will be a $4.00 per hour fee for each child in attendance.

How to Register for our 3K or 4K Programs

1. Fill out and complete the Registration Form and return it to our center with your $25.00 registration fee.  (Space is limited, return your form early.)

2. Complete the “Health History” and “Emergency Care” Forms for your child, indicating if he or she has any known allergies we should be aware of.

3. Indicate on the form an emergency contact number we can reach you at during center hours, and also which 3 people are allowed to pick-up your child in case you are unable to do so. 

4. Have your physician complete and sign your child’s Physical form, indicating they are healthy and can be in a group setting. 

5. Have your physician either give you a copy or fax a copy of your child’s immunization records over to our center. 

6. We will also need to see an original CERTIFIED copy of your child’s Birth Certificate, verifying their age. 

7. Plan on attending and bringing all the completed forms to the St. Lucas School Registration Day in early August.   At this time you will be able to set up the FACTS payment arrangements, pay for your child’s monthly hot lunch and milk. It will also be a time to meet other preschool and elementary school families and pick-up any additional preschool information.  Go to www.saintlucas.org/school/monthly-calendar to find out the exact date and times of the registration day.   

8. All enrolled parents are expected to attend the registration day in order to be a part of our preschool program.  Failure to complete and return appropriate paper work may result in loosing your child’s seat in our program.  


What Do I Bring to Preschool?


• A regular sized backpack to take home papers and notes.
• An extra set of clothes in a zip-lock bag, labeled with your child’s name on it. (Include a shirt, pants, socks, and underwear)
• A blanket or sleeping bag and a small pillow for rest time. (Put your child’s name on each one.)
• A Lunch Box if you are not participating in the hot lunch program. (With your child’s name on it) Our center will provide a morning and afternoon snack for your child, as well as juice and milk daily.

Pick-up Time
 
Three people, other than the parents or guardians, may be authorized to pick up your child.  In case of an emergency, you must notify the Center if an unauthorized person is picking up your child. Please advise the pick-up person to bring in their Driver’s License so the teacher can verify the person picking up your child.  No child will be allowed to leave with any individual without the proper identification. Please realize that the teachers are instructed to be sure about the pick-up authorization.  This is for the safety of your child
 At departure time, a parent or authorized person must sign the child out and make sure the teacher is aware of the child’s leaving.  .

Late Fees for picking up your child after center is scheduled to close.

 Any parent or guardian arriving after 6:00 p.m. will be charged $1.00 per minute for each child in attendance past 6:00 p.m
This fee must be paid to the center before leaving with your child for the evening.  If this is not done, then an additional late fee of $10.00 may be added to your next billing plus the amount you the center for the late pick-up.  To avoid such fees, please arrive no later than 5:50 p.m.so you can gather up your child’s belongings and leave before the center closes at 6:00 p.m.
